Terms And Conditions

  • An informed consent and release of liability waiver must be signed upon arrival.
  • I understand that to make a reservation I need to give a deposit of 10% or cost of the program (whatever is lower) and that is non-refundable after 72 hours, but it can be applied to an upcoming program within a year from cancellation.
  • Any remaining balance is due 2 weeks prior to the program start date, this balance is non-refundable but may be transferred under special circumstances.
  • If hospitalized, it must be 3-4 weeks post-hospitalization before being accepted to participate in any program.
  • I must be capable of self-care including eating, showering, dressing, walking, administering medications, and glucose monitoring to myself, etc.
  • I understand if assistance in normal self-care is needed, I am responsible to bring an assistant to help me during the program.
  • I attest that my health condition is true to the best of my knowledge.
  • I understand if my health condition is not true to the stated facts the session date may be terminated.
